Submission of Results

PATIENT CONSENT FOR THE RETENTION AND PROCESSING OF PERSONAL DATA UNDER THE GENERAL DATA PROTECTION REGULATION (GDPR)

I hereby give my consent to the Diagnostic Laboratory “MICRODIAGNOSTICS S.A.” for the processing of my personal data in accordance with Regulation 2016/679, as outlined below: The sole purpose of processing personal data by MICRODIAGNOSTICS is to provide the services requested by the patient. The collected personal data ensures the proper traceability and handling of the diagnostic sample and its processing results. All personal data is processed by duly authorized personnel, with measures in place to ensure data protection through the implementation of security protocols. The
collected data is not subject to further processing beyond the purpose for which it was collected. If the patient does not wish to provide the personal data necessary for the examination, MICRODIAGNOSTICS will be unable to offer its services.

The necessary personal data we collect includes: full name, ID card/passport number, email address, postal address, telephone numbers, medical history data, age, gender, Social Security Number, and any other data deemed necessary on a case-by-case basis. This data is not shared with third parties except for insurance funds, and it is retained for a period of ten years. The patient has the right to submit a written request to MICRODIAGNOSTICS via email at dpo@microdiagnostics.gr for the modification, deletion, or restriction of their data, as well as for any inquiries.

By signing this form, I also DECLARE RESPONSIBLY that I wish to receive my test results at the email address provided above, as well as at the address of my attending physician/clinic where my biological sample was collected.
